Join us for a community veterinary care program service day with pet vaccinations and health check-ups. Professional advice and personalized care are available for each pet registered. Location address given after completed form. Your will get a confirmation by phone and email leading up to the event as reminder.

As a requirement for participation in this program, CCAWL must collect information through an online digital registration form. Your

response to all questions is required for CCAWL and the licensed veterinarians to provide services. The information you provide here will be used solely for grant and sponsorship reporting, allowing us to continue providing these services to our community. Your responses will be anonymous.
